Employee Equipment Agreement Form

In acceptance of these devices, you acknowledge and agree to the following conditions: 1. You are responsible for the care and safekeeping of all company-issued equipment. 2. Equipment must be used primarily for work-related purposes. 3. You must not modify, disassemble, or alter any company equipment. 4. Any damage, loss, or theft must be reported to management immediately. 5. Equipment must be returned in good working condition upon termination of employment. 6. The company reserves the right to inspect equipment at any time. 7. Software installations must be approved by IT management. 8. You must comply with all company IT security policies. 9. Personal data stored on company devices may be subject to monitoring. 10. Failure to return equipment may result in deductions from final pay as permitted by law. 11. You must back up work data regularly as instructed by IT. 12. The company is not responsible for personal data stored on company equipment.
